East Hill Medical Center (EHMC) sees value in offering patient education and in-home assessments through our partner Cayuga Community Health Network (CCHN).  We know your time and your health are valuable and we hope that you will consider participating in the Home Health Connection Program. Our goal is to help you reestablish connection to health care, assess health risks, access resources, and provide you with the best care possible.
 
What to Expect
CCHN Staff will conduct a home visit to provide non-clinical environmental services related to social drivers of health, lifestyle and other aspects of care management services in collaboration with EHMC. CCHN will assess your risk related to slips and falls, chronic disease self-management, asthma and other conditions. CCHN may provide referrals and help make appointments to address identified needs.
Confidentiality
Information gathered by CCHN during their home visit may be shared with EHMC to provide you with more comprehensive care. Documentation gathered may be added to your file on record at EHMC as appropriate. However, as your primary care provider, EHMC will not share your personal health information with CCHN.
