Sliding Fee Discount Program

 

East Hill Medical Center offers a sliding fee program that allows us to adjust the charges for medical and dental services for those patients who qualify, based on their household income and family size. We will adjust only the portion that a patient is responsible for paying, except insurance co-payments.

If you are interested in applying for this program, please contact our Billing Office at 315.253.8477. You can also ask for an application the next time you are in one of our offices. Verification of all income sources must be submitted with the application. Acceptable verification includes a copy of your most recent income tax return, last two pay stubs from your employer or unemployment agency, or statements from public assistance or Social Security, etc.
