Open Positions

Lead Patient Care Navigator

Identifies care navigation needs and implements effective action plans for the patient who may have :

  • Behavioral health conditions

  • High cost/high utilization

  • Poorly controlled or complex conditions

  • Social determinants of health and/or other barriers to care

  • Referrals to or from outside organizations (e.g., insurers, health system, ACO), practice staff, patient/ family/caregiver

  • Care gaps and preventative health needs

  • Special population status with unique needs

  • Other identified circumstances/needs

· Will assist with patient visits to support the patient and practitioner needs.

· Acts as a patient advocate internally and with outside agencies.

· Establishes person-centered action plans for patients identified for care navigation services and communicates the plan effectively to the patient/family/caregiver.

· Identifies patients with unplanned inpatient admissions and ER visits shares clinical information and ensures contact with patients/families/caregivers for follow-up care.

Medical Assistant

East Hill Medical Center is looking for a Medical Assistant to join our growing organization. This position is primarily located in our Adult Medicine department, but the candidate should be comfortable floating to all our medical offices (Summit Pediatrics, Duckett Family Medicine and Adult Medicine). Our Medical Assistant will assist providers and nurses with the scheduling and flow of patients and patient’s family in the clinical areas. The schedule for this position is Monday-Thursday and hours to be determined. Our Adult Medicine office is open from 8 a.m. to 8 p.m. Monday-Thursday, so the shift would fall within those hours. 9 observed holidays off, weekends off, and 2 additional floating holidays.

Responsibilities:

  • Responsible for supporting and monitoring patient flow in the front and back offices

  • Responsible for performing various aspects of patient visit

  • Reviews health record with the patient

  • Obtains vital signs and patient data

  • Initiates the follow-up of specialty care referrals and services per policy

  • Assists patients with the completion of medical forms

  • Responsible for performing important medical tests and procedures on patients, as directed by practitioner within the MA scope.

  • Performs laboratory specimen collection, preparation and labeling

Dental Assistant

East Hill Medical Center is seeking a full-time Dental Assistant, certified preferred, but willing to train the right candidate looking for a new career path. This candidate needs to be comfortable assisting the Dentist with materials and instruments that are needed for procedures; greeting patients and reviewing their personal, family, social, and health history; taking bitewing and panoramic x-rays; sterilizing instruments and sanitizing patient care areas; stocking tracking materials and equipment. Will also act as back-up reception.

We offer competitive wages, paid time off (including PTO, 9 Observed Holidays, and 2 Floating Holidays). We also offer medical, dental, and vision coverage (eligible the first of the month after hire). East Hill Medical Center matches up to 4% of employee’s contribution.

Schedule: Monday-Thursday, 10 hour shift (40 hour work week)

Dental office open Monday-Thursday 7:30 a.m. to 6:00 p.m.

Dentist

  • Examines patients to diagnose and treat oral and dental conditions.

  • Provides routine care of patients including but not limited to; Examination of head and neck; administration of medication; periodontal therapy; x-rays and follow up maintenance checks.

  • Management of emergencies including but not limited to; control of acute hemorrhages, anaphylaxis, respiratory arrest, cardiac arrest and seizures.

  • Diagnosis, treatment and management for illnesses and diseases that present in the office.

  • Conducts procedures related to routine care or to symptoms exhibited during office visit including but not limited to; Restorations, non-surgical extractions, pulpotomy, dentures, and incision and drainage.

  • Provides consultation with specialist regarding administration of care to patient as applicable.

  • Documents care provided in the Electronic Medical Record.

  • Participates in Quality Improvement initiatives for the dental office including peer review.

  • Maintains confidentiality of all patient information.

LPN

East Hill Medical Center is seeking a full-time LPN for our growing organization. The candidate will be based in one of our medical offices (Adult Medicine, Duckett Family Medicine, Pediatrics, or Inner Wellness), and may be required to float as needed for coverage. Hours are Monday-Friday various hours 8, 10, and 12 hour shifts available. Some evenings may apply based on the provider schedule, with the potential to adjust hours in the future. The ideal candidate will be compassionate and strive to provide an exceptional experience for each patient, every time.

Requirements:

  • Graduated from an accredited school of Practice Nursing.

  • Valid NYS license to practice practical nursing.

  • 1+ years’ experience as an LPN, preferred (New grads encouraged to apply!)

  • Medent experience, is a plus.

  • Prior experience working in an office setting with behavioral health, pediatric and adult patients, preferred.

  • Current BLS Certificate, required.

  • Current Infection Control Certificate, required.

Public Health Specialist

This role is going to serve as a program support and backup for a street outreach program. The Public Health Specialist will assist in the launch of a collaborative program to meet patients where they are, and support them in social needs, including connection to substance use programs and health care to achieve holistic health outcomes. This position is responsible for assisting in developing, promoting and monitoring a NYS OASAS funded street outreach program focused on outreach, engagement and harm reduction. The specialist will serve as a liaison with the collaborating SUD provider including partnering certified peers, and other community resources.

  • Assists the clinical team in the provision of services to patients in the field and through East Hill’s practices, particularly in helping with coordination with outside organizations.

  • Assist in information gathering.

  • Lead data collection efforts, and assist with data analysis and tracking for program reporting.

  • Coordinate efforts to design and implement initiatives and workflows

  • Partner with SUD peers and a clinical partner in home visits and field visits for the unsheltered population. Assist in successful enrollment in care and referrals, including assisting with telehealth connections.

  • Compile and maintain regular reports based on internal or funders’ needs

  • Coordinate with the communication team to facilitate public outreach and education efforts
