Open Positions

Receptionist

  • Full time receptionist position for medical office (Adult Medicine department).

  • Monday - Friday (5) 8 hour shifts from 8 a.m. to 4:30 p.m.

  • Floats between all offices as needed and directed by the Practice Manager or designee.

  • Provide excellent patient service.

  • Greet patients in a friendly and welcoming manner both in person and on the phone.

  • Promptly and professionally answers all incoming, multi-line, telephone calls ensuring to introduce yourself and the practice. Screen and direct calls appropriately.

  • Monitors provider’s schedules to ensure it is efficiently being booked.

  • Schedules and reschedules patient appointments over the telephone, in-person and follow up visits at the end of the initial patient appointment.

  • Assist patients in scheduling appointments and maintaining records and accounts.

  • Assisting patients in getting rostered to our practice as their Primary Care Provider (PCP) if they are not assigned to East Hill.

  • Verifying insurance prior to their appointment.

  • Reminding patients to arrive early if documentation needs to be completed prior to their appointment.

Medical Records Associate

  • Respond within set timeframe to requests for transfer of records that align with state and federal regulations.

  • Sort documents received by fax and mail, then index them consistently according to set standards in electronic medical records.

  • Coordinate with companies to schedule times for bulk file reviews.

  • Communicate with referring agencies, including but not limited to long-term care homes, specialists’ offices and other practices regarding contact information or document requests.

  • Respond to legal requests for information as appropriate or provide requests to clinical director.

  • Scan clinical and financial patient documents into the electronic medical records systems.

  • Maintain paper records in accordance with state and federal regulations, as well as company policies.

  • Ensuring that medical records are organized, accurate, and complete.

  • Ensure compliance with HIPPA and privacy laws regarding patient documents.

  • Prepare document for signatures from physicians and Chief Medical Officer.

  • Receive and route insurance documents to appropriate departments.

Family Nurse Practitioner

  • Works as part of a team, using a participatory style of service delivery, to provide quality nursing care to all EHFM clinic patients.

  • Documents and maintains health care records in EHFM’s electronic medical record system.

  • Initiates a comprehensive, ongoing identification of patient needs in order to develop and implement a plan of services and advocacy for that patient under the direction of the Clinic Director.

  • Follows case management plans and procedures per EHFM’s Administrative and Clinical policy and procedures manual.

  • Serves as the primary provider. Provides patient diagnosis, treatment, consultation, and follow-up under the supervision of one or more physicians.

  • Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.

LPN

East Hill Medical Center is seeking a full-time LPN for our growing organization. The candidate will be based in one of our medical offices (Adult Medicine, Duckett Family Medicine, Pediatrics, or Inner Wellness), and may be required to float as needed for coverage. Hours are Monday-Friday various hours 8, 10, and 12 hour shifts available. Some evenings may apply based on the provider schedule, with the potential to adjust hours in the future. The ideal candidate will be compassionate and strive to provide an exceptional experience for each patient, every time.

Requirements:

  • Graduated from an accredited school of Practice Nursing.

  • Valid NYS license to practice practical nursing.

  • 1+ years’ experience as an LPN, preferred (New grads encouraged to apply!)

  • Medent experience, is a plus.

  • Prior experience working in an office setting with behavioral health, pediatric and adult patients, preferred.

  • Current BLS Certificate, required.

  • Current Infection Control Certificate, required.

Public Health Specialist

This role is going to serve as a program support and backup for a street outreach program. The Public Health Specialist will assist in the launch of a collaborative program to meet patients where they are, and support them in social needs, including connection to substance use programs and health care to achieve holistic health outcomes. This position is responsible for assisting in developing, promoting and monitoring a NYS OASAS funded street outreach program focused on outreach, engagement and harm reduction. The specialist will serve as a liaison with the collaborating SUD provider including partnering certified peers, and other community resources.

  • Assists the clinical team in the provision of services to patients in the field and through East Hill’s practices, particularly in helping with coordination with outside organizations.

  • Assist in information gathering.

  • Lead data collection efforts, and assist with data analysis and tracking for program reporting.

  • Coordinate efforts to design and implement initiatives and workflows

  • Partner with SUD peers and a clinical partner in home visits and field visits for the unsheltered population. Assist in successful enrollment in care and referrals, including assisting with telehealth connections.

  • Compile and maintain regular reports based on internal or funders’ needs

  • Coordinate with the communication team to facilitate public outreach and education efforts

Part-Time Custodian (2 Positions) 

This position is a cleaner is responsible for all basic and deep cleaning in and around the clinical and administrative areas of two buildings. Tasks may entail dusting, mopping, sweeping, vacuuming, sanitizing exam rooms and clinical spaces, and wiping smudges off windows and doors. Ensuring restrooms are cleaned, sanitized, and restocked is another important responsibility of a cleaner. This person will work with another part-time cleaner during the evenings, 4:30 through 9:30 pm Monday through Friday, 25 to 28 hours a week. Primary functions include:

  • Clean clinical and office spaces in two Auburn-based buildings in the evenings

  • Perform cleaning activities as needed such as dusting, sweeping, vacuuming, mopping, sanitizing, removing trash, etc.

  • Stock and supply designated facility areas, such bathrooms and clinical areas with soap, towels and garbage bags

  • Ensure spaces are prepared for the next day by taking out trash, tidying furniture and dusting surfaces
