East Hill collaborates with local partners to provide essential services to the community. These partnerships help us address community needs, promote wellness, and ensure that individuals and families receive the support they deserve.

Harm Reduction Street Outreach Program

Substance Use Disorder (SUD) is a disease, not a choice. It affects anyone. Our community has done a lot to combat fatal overdoses, but people are still suffering.

With funding from the NYS Office of Addiction Services and Supports, East Hill Medical Center and Nick's Ride support individuals via certified peers who provide compassionate, judgment-free care to those in any stage of addiction.

The Harm Reduction Street Outreach Project offers peer support, navigation to medical assistance, and essential supplies, including harm reduction tools, warm clothing, and personal care items. Outreach focuses on individuals and families who struggle to access treatment, recovery, or harm reduction services.

Participation in treatment is not required.

Occupational Health

East Hill Medical Center provides a variety of services tailored to support local businesses and organizations, including DOT physicals and general work physicals.

 

EHMC also partners with multiple fire departments and law enforcement agencies to deliver annual physicals for their members and staff.

Home Health Connection

East Hill Medical Center (EHMC) sees value in offering patient education and in-home assessments through our partner Cayuga Community Health Network (CCHN). 

 

We know your time and your health are valuable and we hope that you will consider participating in the Home Health Connection Program.

Our goal is to help you reestablish connection to health care, assess health risks, access resources, and provide you with the best care possible.

This project is being supported, in whole or in part, by federal award number #21.027 awarded to the City of Auburn by the U.S. Department of the Treasury. 

  • What to Expect
    CCHN Staff will conduct a home visit to provide non-clinical environmental services related to social drivers of health, lifestyle and other aspects of care management services in collaboration with EHMC. CCHN will assess your risk related to slips and falls, chronic disease self-management, asthma and other conditions. CCHN may provide referrals and help make appointments to address identified needs.
  • Confidentiality
    Information gathered by CCHN during their home visit may be shared with EHMC to provide you with more comprehensive care. Documentation gathered may be added to your file on record at EHMC as appropriate. However, as your primary care provider, EHMC will not share your personal health information with CCHN.
