Behavioral Health
Behavioral Health services are available to all enrolled medical patients of East Hill Family Medical, Inc., including children over the age of seven that are seen by pediatricians at Summit Medical. All patients, adult and children need a referral from their primary care doctor to be seen by behavioral health providers.
These services are offered by a Licensed Clinical Psychologist and a Licensed Clinical Social Worker with an R number.
Behavioral Health Services may include (but are not limited to ) individual therapy for adjustment disorders, anxiety, depression, grief and loss issues, symptoms related to post traumatic stress, and relationship conflict.
Behavioral Health services also offers case management services. Our case manager assists East Hill patients with medical referrals, and provides advocacy, support and outreach to other community agencies according to the patient's needs.
We do not provide ADHD evaluations, or evaluations/testimony for court cases.
We accept most insurances including Medicaid managed care.
A clinical psychologist and certified social worker are available to provide counseling services to adult medicine patients of East Hill Family Medical, Inc. We provide counseling services to all East Hill patients when referred by their medical provider.
