If you are interested in becoming our patient we ask you to call the department where you would like to be seen:

You will be asked for your address, telephone number and insurance information. If you are transferring from another doctor or if you have a follow up from the emergency room/urgent center, you will be asked to have these records sent to East Hill before your appointment so that our medical staff can provide you with the appropriate follow up care.
East Hill Family Medical participates with most major insurers, including Medicare and Medicaid. Please bring your insurance card with you for every visit.
Any person without insurance will be asked to call East Hill’s Facilitated Enroller at 315-253-8477 press #1. Our Facilitated Enroller will review all medical insurance options available to you and assist you in the process of enrolling into a medical insurance plan. You may also be given the option of applying for the fee scale program from EHFM.
You are responsible for paying for non-covered services, co-payments or unmet deductibles in full at the time of your visit. Payment in full is also expected at the time of your visit if EHFM does not participate directly with your insurance plan. At the time of your visit, you may pay by cash, personal check, or credit card—Master Card or Visa.
At Your First Appointment:
- Come 15 minutes early to complete all new patient paperwork.
- Bring a picture ID with you [student card, drivers license, passport].
- Bring in your insurance card and be prepared to pay for any non-covered services.
- Bring a list of your medications or bring your medications with you.
- If you have completed a health care proxy please bring it in so we can make a copy.
